INHERENT RISKS WARNING AND WAIVER FORM

ZIPLINE MTL SPECIFIC REGULATIONS

  1. It is dangerous and prohibited to be under the influence of drugs or alcohol for this zip lining activity.
  2. Smoking is strictly prohibited on the entire site.
  3. The use of cell phones or cameras is prohibited during the activity, unless authorized by management.
  4. It is mandatory to have only one (1) person per zip line.
  5. Tyrolienne MTL management reserves the right to exclude any person not adhering to safety instructions without prior warning and without obligation to refund entry fees.
  6. All necessary measures and personnel for your safety have been put in place. You must adhere to these rules which have been clearly explained to you.
  7. It is strongly recommended to tie back your hair and wear sportswear during the activity. We disclaim all liability for stains and tears on clothing and shoes.
  8. Tyrolienne MTL management reserves the right to interrupt the activity if weather conditions require it.
  9. Once payment has been made, no refunds will be issued, regardless of the reason.

INHERENT RISKS

I acknowledge having been informed about the inherent risks associated with the activity provided by Tyroliennes K-DOT Inc. The risks associated with the activity include, but are not limited to:

  • Injuries resulting from falls or other movements (fractures, sprains, bruises)
  • Stress-related disorders from the activity (dizziness, hyperventilation, palpitations and/or increased heart rate, shortness of breath, vertigo, etc.)
  • Burns or heat-related disorders (heatstroke, sunburn, heat exhaustion, etc.

Individuals who are overweight (over 250 pounds) and/or in poor physical condition should be aware of the specificities of these activities and may choose to abstain if necessary.

CONFIRMATION OF INFORMATION AND ACCEPTANCE OF RISKS

JI confirm the accuracy of the information provided in this form to the best of my knowledge and declare that I have not knowingly omitted any relevant information regarding my health. I understand that the details provided here are confidential and serve to ensure the safe planning of the activities in which I will participate. Having been informed of the risks, I acknowledge being aware of the inherent dangers associated with these activities and agree to undertake them fully aware of the associated risks. Furthermore, I commit to actively manage these risks by adopting a proactive attitude towards my safety and that of other participants.

The organizer reserves the right to exclude any individual deemed to pose a risk to themselves or others.

WAIVER OF MATERIAL RESPONSIBILITY

I, the undersigned, hereby waive any claim, as well as any pursuit for damages, for any damage caused to my personal belongings and materials, including normal wear and tear, loss, breakage, theft, and vandalism. As a result, I absolve Tyroliennes K-DOT Inc., the Société du Vieux-Port de Montréal and its subsidiaries, as well as the organizers of this activity from any responsibility.

I acknowledge having taken all necessary measures to secure my personal effects such as cell phone, keys, boots, and others. Consequently, I release Tyroliennes K-DOT Inc., the Société du Vieux-Port de Montréal, and their subsidiaries from any liability in the event of an incident related to the fall of these objects, as well as injuries resulting from their fall or breakage.
